can someone direct me to a webpage for the outboard base (with witnesses) installation / repair?

Cover the bolts with your rubber sealant and drag them to through. Inside put some rubber washers and bolts down. Run your cables and connect the controls. Then see if the former check box holes align. I, not I generally use one of the old holes and drilling a new bolt for my box control.
